The state-level example forms for the various steps of the process are included in the School-Level Planning and Implementation Guide [doc], which can make it easier to enter and share the information with colleagues and administrators. The five components of the evaluation cycle are
- self-assessment;
- goal setting and educator plan development;
- plan implementation and evidence collection;
- formative assessment/evaluation;
- and summative evaluation.
